The template on this guide page is the most current and accurate version of the template. I make changes to it frequently. Seriously--do not trust any template given to you by someone else. Use the one on this guide page.
If you use a citation manager tool, do not trust that it is 100% accurate. You need to check each and every citation to make sure it follows the rules.
